IFSP Checklist for Providers: During the Meeting

The IFSP, or Individualized Family Service Plan, is the written early intervention plan for a child from birth to age three. Use this checklist during the meeting to keep that plan family-centered and legally sound for infants and toddlers who are deaf or hard of hearing. It prompts you to support genuine family participation, share assessment findings alongside everyday observations, write goals that are functional and measurable, spell out each service clearly, and confirm everyone's role on the team. Use it so the finished plan reflects the child's development, the family's priorities, and real communication access.
