Initial IFSP Checklist for Families: Before the Meeting

After a child is identified as deaf or hard of hearing, families may meet with an early intervention team to build an IFSP, or Individualized Family Service Plan, a written plan for children from birth to age three that covers both the child's development and the family's priorities.
