Home-Based Services Provider Checklist

Home-based providers support Deaf and hard of hearing children and their families during everyday routines, right where they live and play. Use this checklist to reflect on family-centered, language-rich practice on each visit. It asks whether you build communication into daily routines, coach caregivers to keep learning between visits, support families in learning ASL, and connect them with Deaf adults, mentors, and community events. It also covers using observation to guide next steps and documenting progress toward each child's IFSP, the Individualized Family Service Plan that lists the family's goals and supports.
