Steady check-ins keep services responsive as a child grows and a family's needs shift. Use this checklist at each IFSP review, or at least once a quarter, to track progress for a Deaf or hard of hearing child in a birth-to-three program. It covers updated language levels, hearing technology and audiology, service delivery including any gaps and whether visits happen at home, in a center, in the community, or by telehealth (video visits), family engagement and confidence, and any access or equity barriers.
