Interdisciplinary Collaboration Provider Checklist

Interdisciplinary collaboration means the partners in a child's care, including home-based, center-based, medical, education, and Deaf community, working as one coordinated team. Use this checklist to do your part across both home and center settings. It asks whether you share observations to support integrated planning, use consistent language and program-approved materials so families get the same message, and coach caregivers in both settings. It also covers connecting families with Deaf adults and visual language access, and communicating barriers and progress back to the team so services stay coordinated.
