Tip Sheet: Transition and Exit Planning for Birth-to-Three Programs

A thoughtful exit honors a family's journey and sets them up for what comes next. Use this tip sheet to build a consistent, family-centered exit process in a birth-to-three program. It offers five practices: an exit survey in the family's preferred language; a plain-language family report that leads with what the child can do; transition planning with the next school's contacts and a plain-language summary of the family's rights; connecting each family to the Deaf community before they leave; and tracking program outcomes with a shared template.
