EI Data Collection Checklist: Enrollment

Good data starts on day one. Use this checklist when a family first enrolls to build a complete baseline for each Deaf or hard of hearing child in a birth-to-three program. It covers child and family demographics, hearing technology, a communication and language starting point, referral and service history, and the family's own priorities and connection to the Deaf community. Staff closest to the family collect the information, and directors make sure it enters program-level tracking. Use it so you know who you serve, can spot access gaps, and can measure language growth over time.
